We are in search of a professional, compassionate, and dynamic Licensed Practical Nurse (LPN) to join our Fellowship Square team. The candidate should be devoted to enhancing the lives of our Assisted Living residents, dedicated to addressing their daily needs, and committed to maintaining their dignity and quality of life. 

The purpose of the Licensed Practical Nurse (LPN) position is to provide wellness services to the residents in the Oasis Assisted Living facility in accordance with the current applicable Federal, State, and local standards, guidelines, and regulations and our established policies and procedures.

Schedule: Tues to Sat, 7am-3pm

• Assist with the preparation of surveys and audits, as well as the associated record-keeping tasks throughout the year.
• Serve as the primary staff member responsible for ordering medication refills and monitoring cycle fills, with the support from the Oasis Management Team.
• Administer TB testing for residents.
• Serve as the Oasis Nurse and Medical Supervisor, to conduct training for Caregiver staff on practical tasks. These include, but are not limited to, Foley catheters, medication administration including subcutaneous insulin, gait belts, and other related Assisted Living equipment.
• Conduct monthly visits to each resident's apartment, assessing them for any changes in condition and addressing additional needs or concerns.
• Ensure the confidentiality of all pertinent resident and employee information to protect their rights.

• Must be a graduate of an approved school for a Licensed Practical Nursing or Licensed Registered Nursing for the state of Arizona.
• Must pass a pre-employment drug test and background check.
• Ability to follow written and verbal instructions.
• Experience in an assisted living or long-term care environment preferred.
• Must be motivated, self-starter, and with a passion for serving seniors.
